- Part-time (20 hours/week) remote Hospice Physician
- As a Physician in this role, you will be assisting with hospice starts of care
- As the initial contact from nursing, you will determine hospice eligibility, evaluate patients for qualifying terminal diagnosis, and write the initial Certificate of Terminal Illness
- Collaborate with on-site team to develop personalized treatment plans and determine appropriate goals of care
- Manage the ongoing care of patients, and monitor progress
- Provide symptom management, medication stewardship, and controlled substance monitoring
- Average admissions range from 7 – 14 per day
- You will work collaboratively with experienced, compassionate staff, and a team of nurses and specialists at each location
- EPIC EMR
- Be part of an integrated nationally recognized organization with physician-led medical group and embedded service line
- Leverage access to DAX Copilot, an AI-powered clinical documentation tool integrated with Epic, streamlining clinical workflows by generating draft patient notes for efficient review, editing, and signature
- Experience an environment that provides safe and equitable care for all patients
- Completion of an accredited Residency Program
- Completion of an accredited HPM fellowship is preferred, not required
- If not fellowship trained, hospice or palliative experience is required
- Compassionate physician with a strong understanding of the hospice philosophy of care and expertise in hospice medical management
